Employers often send OSHA a dump of random PDFs, screenshots, old safety manuals, missing training sheets, unsigned forms, or documents that do not match the actual jobsite. Sending incomplete, outdated, mislabeled, or contradictory documents can create more questions than answers.

Waiting too long to respond

  • Some employers freeze after OSHA contacts them. They avoid calls, delay document production, or hope the issue goes away. That usually makes the situation worse. For federal OSHA complaint investigations, an employer response can affect whether OSHA conducts an on-site inspection, so the quality and timing of the response matters.

Trying to “fix” documents after the fact

  • Backdating training records, creating fake inspection logs, rewriting safety programs to look like they existed before the incident, or filling in old forms after OSHA asks for them can create serious credibility problems.

Not knowing the scope of the inspection

  • Employers often fail to clarify why OSHA is there, what triggered the inspection, what location or hazard is involved, and what records are being requested.

Missing abatement deadlines

  • After citations are issued, employers may need to certify abatement or submit additional documentation. Missing abatement requirements can create more problems after the original citation.

Written programs that do not match the workplace

  • This is very common. A company may have a generic safety manual, but no site-specific plan, no proof of training, no competent person records, no inspections, or no evidence that employees were actually following the program.
